How to add family member

It's easy to add your partner or children* (under 21 yrs old) to your health plan.

 

All you need is your details (Name & Policy number) and your partner or the children's* details (Name, Date of Birth, Gender & Relationship to you).

When you add new members to your Plan it may increase the cost of your Premiums.  So we have included the Pricing Tables below to check what your new Premiums may be.

How much extra will it cost?


See the Pricing Tables and some examples below to help you check what your new Premiums may be when you add new members to your Plan. 

Example 1 - Primary Care Extra:

Single Cover 18-45 - when you add your partner to your plan your premiums would increase from $14.01 to $27.63 per fortnightly.

Example 2 - Primary Care Extra: 

Single Cover 18-45 - when you add a child to your plan your premiums would increase from $14.01 to $24.70

Example 3 - Primary Care Extra: 

Two Parent Family 18-45 - when you add another child to your family plan, it will not change your premiums - With our plans you only ever pay for the first child therefore additional children can be added at no cost. 

If you are non-union members, please add extra 10% to the rate.

(*) See your Policy Document for full details.  A parent or other adult who is not recognised as the Member’s partner, cannot be included in the policy as a partner for the purpose of obtaining benefits as a family. Dependent children included in a policy are eligible for benefits up to the end of the year in which they become 21 to 31 December in any year. Family premiums include the cost of the first child, additional children do not incur extra premium costs.
